Ayew joined Swansea in the last summer transfer window and impressed for the Welsh Club, scoring 12 league goals in 34 games
According to media reports in England, Ayew is expected to have his medical this weekend before the move to West Ham.
Swansea have also announced the signing of striker Fernando Llorente from Sevilla.
The Swans will only sanction a move if they sign another striker and are reported to be in talks with Athletico Madrid Borja Baston.
Slaven Bilic has identified the hardworking Ghanaian forward as one who will add quality and thrust to his side as they aim to compete this season.
Ayew has already scored a couple of goals for Swansea in pre-season with boss Francesco Guidolin hopeful that the Ghanaian would stay at the club
